Do you qualify for the CDCP?

You may be eligible if you meet all of the following criteria. If you're unsure, give us a call — we can help you figure it out.

Household income under $90,000
Your adjusted family net income must be below $90,000 per year. Coverage levels vary by income bracket — lower income means higher coverage.

No employer or group dental coverage
You must not have access to private dental benefits through an employer, union, or other group plan. If your spouse has coverage, you may not qualify.
